If you are appealing a denial, insurance companies must make a determination within 45 days and may take one 45 day extension for special circumstances (for a total of 90 days).

Many times, it will involve additional requests for information where they “toll” or pause the clock on the review while you provide the information. While sometimes the requested information is truly necessary, many times it is not. It varies on a case-by-case basis.
