If you are initiating your claim, insurance companies must make a decision within 45 days but can take two 30-day extensions for special circumstances (for a total of 105 days). If you are appealing a denial, insurance companies must make a determination within 45 days and may take one 45 day extension for special circumstances (for a total of 90 days).
